following order form with your complete name, address, and daytime phone
number, as well as the recipient's information. It is imperative that this
information be complete with apartment #, daytime phone number, and zip
code. If any of this information is incorrect or missing it could mean
delaying delivery by one or more additional days, and this is a perishable
